School Application/Release Form To make a copy of our student Application and Release Form click here. It is necessary to fill out this form COMPLETELY, initial where indicated, sign, and date before a student can participate in a lesson at our school. Students under 18 must have a parent or legal guardian fill the form out and sign/date.
